Insurance

I’m in network with most major Wisconsin plans, including Aetna, Anthem BCBS, UnitedHealthcare, Cigna, and ForwardHealth Medicaid. With insurance you pay your plan’s copay rather than the full fee.

Cancellations

Please give at least 24 hours notice to cancel or reschedule. Later than that, a cancellation fee will be applied.

Good Faith Estimate

If you’re paying without insurance, you have a right to a written estimate of what your care will cost, under the federal No Surprises Act. Ask and I’ll provide one.
